Medical Record Requests
Obtain your medical records in person at EMS headquarters or through the City’s FOIA Request Center.
You can obtain your medical record either in person at our headquarters, 4160 Virginia Beach Blvd., or online through the City’s FOIA Request Center.
We do not provide records of minors in person, so you are required to go through the FOIA department. If you are unable to come in person, please go through FOIA.
If you choose to obtain your medical record in person, please be aware that you will need:
- Proof of ID.
If you are trying to obtain a record for someone other than yourself, you need to provide proof of power of attorney or a copy of their death certificate.
