Commercial Ambulance Company Permit Application
Commercial EMS companies wishing to be permitted to operate in Virginia Beach must complete and submit the following application and disclosure form.
The commercial ambulance company permit application process requires:
- A properly completed and submitted permit application and disclosure form.
- Verification that the company’s VA DOH OEMS license is current.
- Verification that the company has no demonstrated pattern of receiving enforcement actions from VA OEMS that would block OEMS endorsement.
- Verification that the principle personnel (CEO/COO/OMD) have no demonstrated pattern of receiving enforcement actions from VA DOH OEMS.
- VDH OEMS Program Representative endorsement that the company is in good standing.
- Verification that the company is in good standing with the State Corporation Commission.
- Verification that the company is in good standing with VB Commissioner of the Revenue and in good standing with VB City Treasurer.
- City Attorney review.
- Approval by City Council.
